Power apps model driven app designer hiding left hand tree on designer surface

Something changed in settings and I'm not sure what happened. Whenever I open a model driven app in any solution in the environment, the tree menu on the left side is disappearing after refresh. See attached video, you can see the tree render then disappear. What setting needs to be adjusted?

During rendering I can see the menu for a moment....

Once its done rendering, it disappears, it's either underneath or pushed off to the side? I can't figure out how to get it back. Many of the options in the ribbon are also not appearing.

Following up, I completely signed out, cleared cookies and signed back in and the menu is rendering properly.

The problem returned, so I logged out of the tenant and logged back in and it is back to normal. I am trying to re-create the issue. If I cannot recreate, I will close.

I believe issue was caused by a network outage, which made part of the 365 authentication timeout. I traced it and the outage was eventually resolved and it is now working again.

I decided to re-open because the problem has returned.

    Some other symptoms:

I get the same behavior when creating a model-driven app in the default environment, so this seems to affect at the tenant level.

I get the same behavior if I go to another internet connection.

I get the same behavior if I switch to another browser (Chrome, Edge)

I am seeing errors in the javascript console that refers to a left nav toolbox.
