Powerapps Custom Input Form connected to Sharepoint

Hi,

I have created a custom Form with alot of Windows in my Powerapps app.

The App should Display our new Ticket system as well as employee onboarding / offboarding.

Here are my 2 Point I have Issues with and cant find a solution:

my 1. Point is that, I´ve created a custom Form. The Employee should type in his issues and all the Information we need to solve the Issue. Sadly I dont get the connection correctly to SP. Every new Problem should create a new Element in Sharepoint.

thats what i found to this topic:

In addition to that, there are some inputs where the User can choose between 4 answers that are given in SP. (For example: wich component does have the Issue ? "Notebook", "Mobile Phone", "Tablet", "Login Issues", "Others")

How can I connect all the Input fields to my Sharepoint List ? / How can I show the already given answers from SP in my Powerapp ?

in addition: I already tried the Datatables and stuff but i wanna create everything myself and design everything myself.
