Flow Won't Get To Switch

HELP! I've been watching and reading so many tutorials but can't seem to get this to work.  Essentially, I have a form that needs the following automation

TRIGGER

Form Question: Is there a cost? (answer Yes/No)

ACTION

Yes - Needs Approval - Approve - Send Email

Yes - Needs Approval - Reject - Send Email

No - Send Email

The PROBLEM is that it doesn't seem to even get to the simple NO of the switch (ie. i'm not getting an email when i test and the form analytics show it has failed).

This is what I've built

Thanks for the screencastify! Really helped to understand the issue.

I see that the form could be simplified to something similar to this:

A condition action could also be used in place of the Switch, as it makes the flow little clearer to read and the fact the Yes/No field only returns a two values: Yes or No. A switch is useful when you have several options to select from.

Can you post any screen shots of the run time and at what point the flow gets to before stopping, and any error messages.

Thank you!  I had tried something simpler and it wasn't working. I can try again 😄

Ok here is the simpler flow as suggested but I am still missing something in the condition.

This is the test form that I used:

    2021-11-11_23-37-13.png

Note the values of the Yes/No field:

    2021-11-11_23-38-00.png

This is the flow I have been using for testing. Note the the text for the condition is case sensitive, so you will need to use Yes as the text for the check (confirm the values used in your form):

This is the runtime screenshot of the flow:

    Hope this helps.

Regarding the other error:  you are using the Send email with options actions, but the options field is empty:

Would the Send an email (V2) be more suited to your needs?

but am still getting an error

I think the error may have something to do with the email piece??
