helpdesk app adding a calendar (date and time)

Hi,

 

I created a simple help desk app and now I have to add a date & time (field name: AvaialbleTime) to the create ticket screen to get the date and time to see the person. 

I want to get the date and time from the app and then save it to the SharePoint list (HelpDesk), and also show it to the Admin Screen.

Please help me
