I have an approval process flow. I have a Start an Approval Action and then a Wait for an Approval action. The Wait for an Approval has a timeout value set in settings.
After the Wait for an Approval I have three parallel paths, one for success, one for skipped and failed and one for timeout. Anytime the Wait for an Approval times out the path followed is "has failed", not "has timed out".

I think the problem is that you are setting the TimeOut based on the scope that contains the Wait for an approval, not on the Approval action itself. In my experience TimeOuts don't roll-up to the Scope level. If an action inside a Scope Times out then the Scope fails.
